I do not recommend a smaller size to minimize heel slipping. You have orthotics to help you with an issue. Squeezing into small shoes will only create entirely new foot problems.
Not all shoes are designed to accomodate orthotics. I reoommend bringing your orthotics to your local professional Shoe Fitter when you purchase new shoes. Their knowledge will help you obtain the best fitting shoes for both the orthotic AND your feet.

Doug #1 is right, squeezing into smaller size shoes may create additional problems. I would suggest bringing the shoes you have been wearing as well as your orthotics in with you when you visit the Shoe Fitter. It is possible that with some modification your shoes may accomodate your orthotics. The Shoe Fitter will be able to determine if the shoes can be made to accomodate the orthotics. A couple of things to look for in new shoes to wear with your orthotics are a removeable insole or footbed so that you can give your orthotic a good solid base and a deep heel counter so that the orthotic doesn't raise your foot up out of the back of the shoe. Hope this helps.

Bring your favorite shoes and any type of insert (custom orthotic or over-the-counter arch support) you wish to use in the shoes and let our professionally trained specialists help.

I agree with my Shoes-n-Feet colleges, going down to a smaller size and/or adding heel grips will not only not solve the problem but can also create other foot problems (i.e. blisters, calluses, corns, etc.). There are a lot of different tricks we Certified Shoe Fitters have at Shoes-n-Feet to make heel slippage go away! First and foremost always wear a shoe that fits your foot and foot shape!
Here are some questions to ask:
Is the inner sole/sock liner of the shoe removable? If so, is the inner sole/sock liner removed before the orthotic is put into the shoe?
If the inner sole/sock liner is still in the shoe try removing it and just put in your orthotic to see if that decreases your heel slippage.
Is your shoe an athletic shoe with some form of motion control?
People who have never worn a properly fitted motion control athletic shoe usually experience a feeling of heel slippage in the beginning. The heel slippage is really a feeling of the shoe doing its job of control foot motion (i.e. controlling when, where, and how much the foot pronates). Heel movement within the shoe is good and healthy as long as it is not excessive. We Certified Shoe Fitters at Shoes-n-Feet usually view anything ¼ of an inch and less as normal.
Does your shoe lace up?
If your shoe is a lace up there are a variety of different lacing techniques that can help minimize heel slippage. Come into to any of our local Shoes-n-Feet stores and we can show you how to tie “The Runner’s Loop” or the “Modified Runner’s Loop.” In fact, there are numerous ways to tie a shoe that can make the shoe fit and feel entirely different!

The shoe fitting professionals at SHOES-n-FEET can also work with your podiatrist to have the orthotics adjusted. I often use a grinder to thin my orthotics at the heel so they will sit lower in the shoe. Along with the shoe adjustments made by the folks at SNF, we can usually eliminate heel slippage quite easily.
